Altruista Health announces that Colorado Access, a safety net health plan in Denver, CO, has gone live on its GuidingCare for Member Services module.

GuidingCare for Member Services is an innovative customer relations management module that provides member services personnel with instant access to care management details as they communicate with patients. By integrating with the GuidingCare platform, customer service representatives can view health improvement opportunities and service utilization patterns. As a result, they become a core part of the overall health engagement and outreach efforts of the plan beyond their ability to address member eligibility, benefits and claims-related enquiries.

In late 2010, Colorado Access implemented GuidingCare, a patient-centric care management system that helps health plans improve health outcomes and reduce avoidable costs associated with populations suffering from or at risk for chronic illness. Guiding Care’s web-based care coordination and automated decision support tools enable Affinity Health Plan’s physicians, nurses, case managers and health educators to provide timely and clinically appropriate care management,  health education, and support services to members with chronic healthcare needs.
