Presentation Focused on How Heath Plans and Providers Can Leverage New CMS Investments by Implementing and Managing Care Coordination Solutions for Medicare Patient Populations

Abraham’s presentation, “Medicare Care Coordination: Expectations, Best Practices and Challenges,” focused on how health plans and providers can leverage CMS’s new investment in funding for care coordination services by implementing and managing these services for their Medicare patient populations. Stella Catalan, Senior Director of Health Services at AlohaCare, A Hawaii-based community health plan and Altruista Health client, also presented in this session.
