Safety-Net Health Plan and ACAP Member Selects Leading Care Management Provider to
Help Improve Health Outcomes, Reduce Avoidable Costs
October 31, 2013 – Reston, VA – Altruista Health, Inc., an innovative provider of care and disease management technology for health plans, providers and coordinated care organizations, announces that they have signed an agreement with Santa Clara Family Health Plan, a not-for-profit health plan dedicated to improving the health of Santa Clara County, CA residents, to implement Altruista’s GuidingCare care management solution.

GuidingCare is a patient-centric care management system that helps health plans improve health outcomes and reduce avoidable costs associated with populations suffering from or at risk for chronic illness. GuidingCare includes several modules that will enable Santa Clara to maximize their limited resources and improve the health of their members, including predictive modeling, gaps-in-care analytics, care management, utilization management, appeals & grievances management and role-based portals for providers and member services teams.
Among GuidingCare’s most valuable aspects for Santa Clara is its UCSD predictive measure set. GuidingCare is the only care management platform to offer this model, which is optimized to help Santa Clara identify high-risk Medicaid-Medicare populations. The model is currently used by the state of California to help drive their risk-adjustment decisions.
