Home Health Care Organization and Health Plan Selects Leading Care Management Provider to Help Meet State of New York’s New Care Coordination Guidelines for Dual Eligibles Programs

Altruista Health, Inc., an innovative provider of care and disease management technology for health plans, providers and coordinated care organizations, announces that they have signed an agreement with Visiting Nurse Service of New York (VNS) to implement Altruista’s GuidingCare care management solution. VNS, a leading provider of home health care services and affordable health plan options for the New York metro area, will implement GuidingCare to help streamline their care coordination workflow, improve outcomes and reduce costs in order to complete their dual-eligibles program demonstration for the state of New York.

New York is one of 26 states that have submitted Financial Alignment Demonstration Proposals to the Centers for Medicare and Medicaid Services (CMS) for improving outcomes and reducing costs of care for Medicare/Medicaid dual-eligible enrollees. As part of these proposals, health plans that manage Medicare and Medicaid enrollees within these states must demonstrate their ability to meet a host of new care coordination guidelines.

GuidingCare is a patient-centric care management system developed to meet the specific care management and coordination challenges of safety-net health plans such as VNS. GuidingCare is the only available care management technology solution to integrate the Chronic Illness and Disability Payment System (CDPS), the most accurate predictive model for Medicaid populations. With more than 300 clinical care, behavioral health and medication-related indicators, GuidingCare will provide VNS with the tools they need to quickly identify opportunities to improve outcomes and reduce costs of care for dual eligibles enrollees.
